How we approach refills — and how to request one

We use the lowest effective dose for the shortest time needed. Here's how to request a refill and how we keep it safe and relevant.

At Fishtown Medicine, we believe that medications are tools, not long-term crutches. Our goal is to use the lowest effective dose for the shortest time needed, and to continually assess whether a medication is still helping, still needed, or worth reconsidering.

What we review before a refill:

Every refill request prompts a quick clinical review, including:

  • Your labs (if relevant)

  • Medical history and risk factors

  • Any changes in symptoms or side effects

  • Whether the medication is still effective or necessary

If there’s a safer, simpler, or more effective option — or if we feel it’s time to consider deprescribing — we’ll let you know, explain why, and walk you through the decision.

When we recommend deprescribing:

If the risk-benefit balance has changed, or if the medication is no longer serving its purpose, we may recommend tapering, switching, or stopping it altogether.

This is part of how we practice safe, intentional, whole-person care — not reflexive renewals.

How to request a refill:

To request a refill, send us a clear photo of your medication bottle or packaging. Please make sure the following are visible:

  • Medication name

  • Dose/strength

  • How often you take it (frequency)

This helps us process your refill quickly and accurately.

If it’s a new medication or one we didn’t originally prescribe, please also tell us:

  • What you’re using it for

  • Who originally prescribed it

  • Whether anything has changed since starting it

If anything is unclear, we’ll follow up. If everything checks out, we’ll confirm once it’s been sent to your pharmacy.
