Sharing Your Medical Records with Us

In order to do so, we encourage you to share any recent labs, imaging, test results, and clinical notes from previous doctors or clinics. These records are an important part of your intake and will be used to guide your care moving forward.

You may be wondering why we ask for these records, and the answer is simple: by reviewing your prior care story, we can:

  • avoid unnecessary repeat testing,

  • spot what's been missed or misunderstood,

  • coordinate better with specialists or past care teams, and

  • safely deprescribe medications that may no longer serve you or address only part of a problem.

We don't just look at numbers — we interpret your records in the full context of your goals, symptoms, and health patterns.

If something in your records is still relevant and accurate, we won't repeat it. However, if something is missing or unclear, we'll explain why a new test might be helpful before proceeding.

You can send your records to us in a few different ways:

  • Uploading them through our secure portal

  • Emailing us directly if we’re texting

  • Letting us know where they’re stored — we’ll request them with your permission
